PGY1 Pharmacy Practice Residency, Pharmacogenomics & Precision Medicine

TRHC is a family of companies that leverages technology to improve healthcare. TRHC works with providers and insurers to identify multi-drug interactions and to reduce risk of medication-related problems. We have developed the first multi-drug interaction tool that identifies risk across a variety of safety factors and presents meaningful opportunities to mitigate that risk. This technology can be used to assess safety at the individual patient-level or to stratify medication risk at the population-level, and can be embedded within any EMR or other healthcare IT system.

The purposes of this Post-Graduate Year 1 (PGY1) residency program of Tabula Rasa HealthCare (TRHC) are to train pharmacists to utilize their accumulated knowledge and experiences in order to practice pharmacogenomics and precision medicine and to excel in their future professional endeavors. The residency year includes required core learning experiences in the following areas: clinical pharmacology & pharmacogenomics, pharmacoinformatics & healthcare technology, geriatric clinical practice, teaching, and research & writing. The residency year also includes elective learning experiences in the following areas: quality assurance-performance improvement (QAPI) also known as total quality management (TQM), and practice management & marketing.

Pharmacogenomics

  • Advocates for the rational and routine use of PGx testing.
  • Recommends PGx testing to aid in the process of drug and dosage selection.
  • Uses informatics to guide recommendations to perform PGx testing.
  • Educates and provides information on the appropriate indications for PGx testing and the clinical application of PGx to health care professionals, patients, and members of the public.
  • Helps coordinate activities of the organization’s PGx clinical services.
  • Aids with ordering, shipping, and tracking of PGx test kits and results.
  • Communicates with PGx laboratories and vendors, as requested.
  • Contributes to analyses & reports provided to leadership, as requested, to monitor and improve clinical services.
  • Participates in the organization’s PGx clinical services.
  • Provides PGx test results interpretation and clinical guidance to prescribers and patients.
  • Makes recommendations to personalize medication therapy based on the patient’s PGx profile that also considers the pharmacodynamic and pharmacokinetic properties of the drug(s) as well as other patient-specific factors such as comorbidities and other concomitant drugs.
  • Follows standardized processes for performing PGx consultations, communicating PGx test results to prescribers and patients, and documenting patient-specific results in the electronic health record(s).
  • Supports and participates in research that drives the application of PGx in clinical practice.
  • Designs and conducts PGx-based research.
  • Participates in the documentation of improved patient outcomes and economic benefits resulting from clinical application of PGx.
  • Actively contributes to the body of knowledge in PGx by publishing articles on the topic in the biomedical literature.
  • Supports and participates in PGx-based CDS.
  • Helps develop PGx-specific algorithms & rules for electronic health record systems that guide prescribers and pharmacists on the appropriate use and dosing of medications based on a patient’s genomic profile.
  • Uses informatics to evaluate the performance of algorithms & rules.
  • Provides effective PGx-related education to patients, caregivers, health care professionals, and students.
  • Designs effective educational activities for patients and caregivers.
  • Develops and plans PGx-specific advanced training opportunities for pharmacists & other health care professionals.
  • Precepts Advanced Pharmacy Practice Experience (APPE) students.
  • Uses effective teaching skills to deliver education.
  • Assesses the effectiveness of education.

Pharmacy Leadership & Management

  • Assists with managing medication-use processes, services and systems, as applicable to the organization.
  • Identifies opportunities and changes needed to improve patient care and/or medication-use systems.
  • Develops plans to improve patient care and/or medication-use systems.
  • Implements changes to improve patient care and/or medication-use systems.
  • Assess changes made to improve patient care and/or medication-use systems.
  • Provides analyses and reports to leadership, as requested, to evaluate outcomes of changes made to improve patient care and/or medication-use systems.
  • Applies a process of on-going self-evaluation and personal performance improvement.
  • Contributes to departmental management.
  • Precepts at least one Advanced Pharmacy Practice Experience (APPE) student.
  • Manages one’s own practice effectively.

Pharmacy Teaching, Research & Writing

  • Provides effective medication- & practice-related education to patients, caregivers, health care professionals, and students.
  • Designs effective educational activities.
  • Uses effective teaching skills to deliver education.
  • Assesses the effectiveness of education.
  • Participates in research initiatives and the enhancement of the organization’s clinical decision support systems.
  • Leads & completes a research-based project that is publication-ready by the end of the residency program year.
  • Works with key stakeholders to disseminate research findings, including presentations at local, regional, and/or national conferences.
  • Conducts literature searches and effectively evaluates scientific evidence.
  • Uses effective written communication to disseminate knowledge.
  • Writes for company newsletters.
  • Contributes to peer-reviewed publications.
