Our personalized Medication Safety Review is the opportunity to prevent harmful drug interactions and find your MedWise Risk Score. Drug interactions may cause adverse drug events (ADEs), the third leading cause of death in the United States. The chance of experiencing an ADE is about 82% when taking seven or more medications. ADEs are expected to reach 200,000 in 2020. Experts at Tabula Rasa HealthCare are dedicated to finding ways to reverse this looming health crisis. With a Medication Safety Review, you can speak with a certified MedWise Advisor™ pharmacist. Our pharmacists use our one-of-a-kind MedWise® Science technology to help identify and reduce ADEs. You can also get a pharmacogenomics (PGx) test with your Medication Safety Review to see how your body reacts to your medications.

What is a Medication Safety Review?

A Medication Safety Review analyzes all of your medications to find potential risks. You’ll speak to a certified pharmacist who uses our MedWise® Science technology. MedWise is the most comprehensive solution to minimize the dangers of ADEs. It is the only technology that cumulatively compares how medications interact together. Using MedWise, the pharmacist can detect possible multi-drug interactions. The pharmacist will help you find a new plan to prevent future risks. We will work with your healthcare team, including doctors and pharmacists. We want to make sure you’re getting the most from your current medications.

What is PGx Testing

PGx testing measures how your body reacts to your medications. Depending on your genetic makeup, some of your medications may work faster or slower. You may also have more or fewer side effects than expected. TRHC partners with OneOme to provide PGx testing. Our pharmacists use PGx test results and practice guidelines to help personalize your medication regimen. You can get a PGx test with your Medication Safety Review for an additional $300.

Should I Get a PGx Test?


You may want to get a PGx test if you are:

  • Currently on or waiting to start clopidogrel or warfarin
  • Taking a medication for depression or anxiety and:
    • Have tried many different medications to control your depression or anxiety
    • Think you may be having side effects, such as tiredness, dry eyes, dry mouth, or weight gain
    • Not responding to therapy
  • Taking an opioid (e.g., hydrocodone, oxycodone, tramadol) for chronic pain and:
    • Think you may be having side effects, such as tiredness or difficulty breathing
    • Do not have adequate pain control in spite of increasing your opioid dose
  • Taking a proton-pump inhibitor (e.g., esomeprazole, pantoprazole, omeprazole) without relief of your heartburn
  • Waiting to start azathioprine or tamoxifen

What Do You Get?

After you speak with a pharmacist, you will receive a Medication Action Plan (MAP) and Medication Schedule in the mail. The MAP reviews what you talked about with the pharmacist. It also highlights safety recommendations to discuss with your doctor. Your Medication Schedule uses colors and pictures to show you when to take your medications. Your doctor will also be faxed an action plan.
